Northampton Carer Form

If you require a carer to support you during your visit, please fill in the form below, stating which date and session time you have booked for.

You MUST bring one of the following with you to allow your carer complimentary entry to the light trail:

• PIP (Personal independence payment)

• DLA – care and/or mobility

• Letter from a medical professional

• D/deaf or blind registration

• The Access Card or equivalent with a +1 Icon for PA tickets

Please note we will not be issuing carer tickets.

Your PA/carer will be granted complimentary entry upon arrival providing you have filled in the form and brought the correct documentation with you. This is granted on the expectation that they are willing and able to assist you throughout your visit, and would be available and able to help you in the event of an evacuation.

We cannot refund any tickets purchased for your PA/Carer.

To apply for a carers ticket:
1 - Purchase your ticket to the light trail (Do not purchase a ticket for your carer, we will issue this at a later date and cannot refund it if you purchase before applying)

2 - Fill out the form below

3 - Bring one of the following to your session

• PIP (Personal independence payment)

4 - Your PA/carer will not be issued a ticket but will be granted complimentary entry based upon you providing the above.
This is on the expectation that your PA is willing and able to assist you throughout your visit, and would be available and able to help you in the event of an evacuation.
